GreenSquareAccord currently deliver a range of services providing a range of information, advice and support to people living with dementia, their carers and families. We currently have a vacancy for a Hard to Reach Groups Support Worker who will be specifically tasked with reaching communities who traditionally do not access support for dementia, cognitive impairment and mental health issues.This post will part of the wider Health and Independence team who deliver post diagnosis support within the community, work within Walsall Manor Hospital, deliver Dementia Cafes across Walsall and our Care improvement Service service that works closely with Walsall care homes.The role is vital in promoting and delivering dementia support to communities that are often under represented or face barriers in accessing service. The role focuses on engaging individuals and communities who may be concerned about memory loss or dementia but face barriers to accessing support or receiving an early diagnosis. The post holder will take the lead in raising awareness about dementia, encouraging engagement, and delivering culturally sensitive group session to individuals and families concerned about a dementia diagnosis.Key Duties:
  • To increase the number of people accessing memory services for formal diagnosis of dementia
  • Raise awareness of the signs and symptoms of dementia, promoting the importance of an early diagnosis
  • Work with local communities and places of worship to raise awareness of the importance of an early diagnosis, by building trust.
  • Engage with communities who may not typically access dementia services, providing reassurance and information in a sensitive and accessible way.
  • To increase the take up of healthy lifestyles as a way of reducing the risk of developing dementia ("healthy body, "healthy mind" "5 Ways to Wellbeing" messages)
  • To deliver a number of dementia awareness sessions across the borough that specifically cater for people from harder to reach communities
  • Liaise effectively with internal teams and external agencies to provide holistic support

  • A good understanding of the needs of people living with dementia and those concerned about memory issues.
  • Excellent communication and time-management skills.
  • Experience working with diverse communities, particularly those who are underrepresented in healthcare services.
  • The ability to deliver group sessions and engage sensitively with individuals who may be anxious or unsure.
  • Strong partnership working skills and confidence in promoting services in the community.
  • Fluency in Hindi, Urdu, Punjabi, or other South Asian languages to support effective communication.
  • A calm, professional manner and an adaptable, person-centred approach
