MAIN DUTIES AND RESPONSIBILITIESWork with the Pharmacist and Clinicians in accordance with agreed practiceprescribing standard operating procedures.Process repeat medication requests, including the conversion of acute requests torepeat medicines, changing quantities of medicines.Issuing prescriptions in line with agreed clinical protocols and within yourcompetencyLiaise with secondary and primary care colleagues to ensure correct medicines arecontinued following the transfer of care.Monitor practice adherence to the repeat prescribing policyTo assist the Pharmacist with data analysis of prescribing behavior in line withNICE guidance and local clinical commissioning directives.To assist with the development and review of medicine auditsTo maintain and develop medicines communication to staffProvide a contact for patients and colleagues with medication queries.Use practice and CCG templates to record near patients monitoring for high riskdrugs.Identify blood tests, recalls and reviews using established protocols and forwardthese to the appropriate clinician.Liaise with Clinicians and staff to resolve prescribing queries.Review hospital discharge notifications and outpatient hospital letters andreconcile medication against current repeats, flagging up and correcting discrepancies.Promote safe use of medication, reporting of medication related incidents andproactively preventing safeguarding incidents.Observe the principles of clinical governance when developing policies, protocols,guidelines, SOPs and procedures.Support practice prescribing budgets by on-going monitoring, audit and analysis of drug use and support practices in reviewing, developing and enhancing cost-effective prescribing. This includes staying aware of latest QIPP and other cost targets.Help review medication with the Pharmacists and make recommendation for prescribing changes to improve outcome and ensure prescribing of the most effective product.Project work/prescribing initiatives under the support of the PharmacistTo adhere to strict confidentiality policies at all times.Attend local meetings seeking to improve access to medicines or repeatprescribing processes.Undertake audits of prescribing as agreed by pharmacistThis job description is not exhaustive and may be adjusted periodically after review and consultation.
