- Four main hospitals: Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest
- Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire
- Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings
- 47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers
- Numerous mental health and learning disabilities services
- Educated to Degree level or evidence of equivalent level of relevant work experience
- Evidence of continuing professional and personal development
- Good understanding of the national and strategic priorities for addressing inequalities in health within communities that are disproportionately disadvantaged
- Good understanding of the cultural, religious and spiritual needs of individuals in diverse communities
- Knowledge and understanding of key legislation relevant to the role and partnership working context e.g. Well-being of Future Generations (Wales) Act 2015 and Social Services and Well-being (Wales) Act 2014
- Certificate of Professional Development/Membership of a professional body
- Knowledge of relevant strategies and guidelines related to health and social care
- To be an experienced community worker understanding difference within diverse communities and their lived experiences.
- Extensive lived experience or experience of working with diverse communities and community networks
- Understanding the issues related to health inequalities
- Experience of joint working with other statutory, voluntary or private sector organisations
- Experience of working to deliver to strict deadlines and working flexibly to meet changing requirements
- Experience of asset-based approaches to community development
- Experience of needs assessment
- Welsh Speaker (Level 1)
- Proactive, self-motivated, able to work on own initiative
- Commitment to own professional development
- Flexible and adaptable approach to work and able to work effectively against a background of change and uncertainty
- Commitment to integrated and multiagency working in order to achieve improvements in the health outcomes and experiences of diverse communities
- Ability to travel throughout the three counties of the Health Board area in a timely manner to urban and rural locations

- Uses reflective practice to improve own performance
