Salary: £28,407 - £34,581 per annumSanctuary Personnel is currently seeking a dedicated Community Nurse to join our client's Integrated Community Nursing Team. In this role, you will provide essential nursing care to patients within the community while upholding the highest standards of professionalism and patient support. This position offers a dynamic opportunity to contribute to the well-being of individuals and families within our community.Key Responsibilities:
- Promoting Positive Image: Serve as a professional ambassador for community nursing, fostering a positive perception of our services within the community.
- Patient Care: Deliver comprehensive nursing care to patients residing in the community, ensuring their physical and emotional well-being.
- Educational Role: Educate patients, their caregivers, and new team members, contributing to the enhancement of knowledge and skills within the community nursing team.
- Maintain confidentiality and adhere to legal requirements regarding data protection.
- Accurately document patient information and participate in team meetings and professional groups as required.
- Identify service gaps and collaborate with the District Nurse Team Leader to address them effectively.
- Safeguard the welfare of staff, patients, and caregivers, adhering to health and safety legislation.
- Participate in the assessment, implementation, and evaluation of patient care plans.
- Assist in care assessments and ensure proper documentation of patient records.
- Facilitate patient education and participate in staff training programs as directed.
- Adhere to policies, protocols, and clinical guidelines to ensure quality care delivery.
- Promote cost-effective practices and efficient service delivery.
- Support induction programs for new team members and contribute to community experience packages for other professionals.
- Maintain professional registration and adhere to the NMC Code of Professional Conduct.
- Engage in personal development activities and participate in supervision and appraisal processes.
- Utilise IT systems effectively for patient records and communication purposes.
- Participate in nursing audits, research projects, and educational activities to enhance patient care.
- Comply with policies and procedures, including risk management, confidentiality, and infection control.
- Uphold the values and vision, prioritising patient care and safety in all actions.
- Registered Nurse with current NMC registration.
- Awareness of current issues in community nursing.
- Ability to maintain NMC registration and participate in mandatory training and development activities.
