With £36.28 per hour and flexible work arrangements, this role is ideal for those seeking meaningful impact and professional balance.Key BenefitsFlexible Working Options: Choose between full-time or part-time hours that suit your lifestyle.Competitive Pay: Earn an attractive £36.28 per hour, recognising your expertise and dedication.Professional Development: Access to ongoing training and career growth initiatives.Diverse Caseload: Work within a fast-paced, collaborative setting that keeps you engaged and constantly learning.Work-Life Balance: Structure your schedule without compromising your commitment to care.Your RoleAs a key member of the Hospital Team, your responsibilities will include:Hospital Discharge Planning: Complete Care Act assessments and arrange safe, timely discharges through the Discharge to Assess (D2A) model.Integrated Working: Collaborate with health professionals, therapists, housing officers, and others to coordinate discharge planning and aftercare.Reablement Focus: Promote independence and recovery, aiming to minimise reliance on long-term care and avoid hospital readmission.Safeguarding Adults: Respond to concerns in accordance with Section 42 of the Care Act and local safeguarding policies.Mental Capacity & Advocacy: Conduct Mental Capacity Assessments and Best Interest decisions; ensure access to advocacy when needed.Care Reviews: Monitor and evaluate care packages for effectiveness, quality, and value for money.Documentation: Maintain accurate records in line with legal and local authority standards using electronic case management systems.What We’re Looking ForQualifications & Registration:Social Work degree or equivalentRegistered with Social Work EnglandExperience:Prior experience in adult social care, ideally in hospital discharge, intermediate care, or reablementSolid understanding of the Care Act 2014, Mental Capacity Act, and NHS Discharge to Assess processesSkills:Confident working in a multidisciplinary teamStrong assessment, planning, and time-management abilitiesExperience with complex decision-making and working under pressureDesirable:Knowledge of Continuing Health Care (CHC) and NHS-funded care pathwaysFamiliarity with Mosaic or similar case management systemsAttributes:Calm, organised, and resilient under pressureStrong commitment to person-centred carePositive, collaborative, and solution-oriented approachWhy Portsmouth?Portsmouth offers a perfect blend of coastal charm, vibrant city life, and rich history.
