This tried and tested method focuses on strengthening our front door and encouraging social care professionals to have meaningful conversations with individuals to create better outcomes. It moves away from process-driven 'hand-offs' and promotes connecting with local networks and community support. By fostering these genuine interactions, we aim to enhance the quality of care and support available in our neighbourhoods and communities.You will need excellent communication, record-keeping, and keyboard skills, along with experience working with individuals who require care and support. A commitment to excellent customer service and the ability to manage a high turnover of work in an organised and efficient manner are essential. You should also have a strong commitment to multidisciplinary working and be familiar with the department's eligibility criteria. Regular individual supervision, peer group support, and access to appropriate training will be provided to assist your learning and development.
