This is an exciting opportunity for a seasoned Social Worker seeking a full-time role to make a real difference in the lives of adults with complex and long-term care needs. We offer the flexibility of working from home with the expectation of attending the office at least once per week. What you will do:
  • Conduct comprehensive assessments for individuals over 18 with complex needs.
  • Develop and implement personalised care plans aiming for positive outcomes.
  • Support individuals with both physical and mental health needs in the community.
  • Regularly liaise with other professionals and agencies to coordinate interventions.
  • Maintain accurate records and prepare reports adhering to high professional standards.
  • Attend scheduled meetings at the office and contribute to team discussions and planning.
  • Provide direct support and advice to individuals and families to promote their overall well-being.
Requirements of the Social Worker:
  • Degree or equivalent qualification in social work.
  • Current registration with Social Care Wales.
  • Substantial frontline experience working in Social Services within the UK.
  • Proven ability to handle complex caseloads and make informed decisions.
  • Strong values and exemplary interpersonal skills.
