Pay Rate: Up to £38 per hourJob Description:Liquid Personnel is seeking a skilled and compassionate Occupational Therapist to join a dynamic Discharge to Assess (D2A) team for its client in Slough.This is a fantastic opportunity to make a real impact by supporting timely and safe hospital discharges, ensuring patients receive the right care in the right place.You will:
- Support with Trusted Assessments as needed.
- Use your clinical expertise to screen and process hospital discharges.
- Facilitate smooth transitions from hospital to home or community settings.
- Work collaboratively with multidisciplinary teams to deliver person-centred care.
- HCPC-registered Occupational Therapist.
- Previous experience in hospital discharge or community rehab settings.
- Strong assessment and decision-making skills.
- Ability to work independently and as part of a team.
