  • Deliver a range of sustainable Healthy Communities projects/programmes that deliver measurable impact for the community across 5 centres in Wokingham Borough
  • Complete any monitoring and evaluation requested by line manager in line with current data protection protocols- including gathering case studies to demonstrate impact
  • In line with relevant training / qualifications held, deliver Places Leisure National Healthy Communities Programmes. This may include: Exercise Referral, Specialist Classes, Falls Prevention, Cardiac Rehabilitation, Cancer Rehabilitation and other programmes which focus on tackling health inequalities.
  • Support the coordination of programmes, including processing referrals from healthcare professionals, conducting telephone consultations, and administration through the bookings system. .

  • Qualified instructor in relevant in relevant discipline to deliver sessions independently i.e. Level 3 GP Referral, Postural Stability Instructor, Level 4 Cardiac Phase IV, Level 4 Cancer Rehabilitation, Level 4 Neurological Conditions
  • Experienced in planning, delivering and monitoring sessions.
  • Passion for improving the health of the local community.
  • Ability to use MS tools
  • Confident interacting with clients in person and via telehone
