There are 10 train stations which serve key neighbourhoods within the borough providing direct connections to London Victoria, London Bridge, London Blackfriars and St Pancras International. Sutton also has direct rail connections with Epsom, Dorking, Guildford and Horsham, as well as Wimbledon for an interchange with the London Underground and West Croydon for the London Overground.   • develop a detailed understanding of the needs for and the availability of accommodation based care and support services in Sutton and out of borough, such as care homes, supported living and shared lives
  • work with Adult Social Care Teams to detail the needs of eligible persons and to advise on accommodation options in accordance with operational procedures such as participating in Consistent Practice Meetings
  • work with Accommodation Based Care Commissioning Managers to agree costs, complete supplier quality checks and seek approval from Adult Social Care Heads of Service and work closely with the Council's Brokerage Service to arrange placements on the Council's case management system
  • work in partnership with health commissioners to support the spot purchasing of joint funded placements, particularly in respect of joint placements under S117 Mental Health Act
  • maintain up to date and accurate records of placement activity and contribute to Accommodation Based Care Commissioning Team's market oversight function, data collection and intelligence gathering
About Us 1The Accommodation Based Care Commissioning Team is responsible for purchasing accommodation based care services for eligible persons. These services include nursing and residential care homes, extra care housing and supported living. Commissioning functions include setting strategic direction, financial modelling, needs assessment, managing demand and market management. The Team operates as part of a wider commissioning function responsible for commissioning a range of care and support services such as home care as well as working closely with Adult Social Care, the Integrated Care Board and the Council's Brokerage Service. The Team consists of three commissioning managers and one commissioning officer.About You 1Any essential qualifications, experience and skills you are looking for in your successful candidateThe successful candidate will have a good understanding of adult social care and experience of working in an adult social care environment or related role. The person will be able to demonstrate good administrative skills, be clearly organised with a keen sense for attention to detail and accuracy of record keeping.
