Job Title: Outreach Family Support Worker Location: Liverpool Contract: 3 months (with the possibility of extension) Hours: Full-time, 35 hours per week (Monday to Friday, standard office hours) Liverpool Early Help is seeking a compassionate and motivated Outreach Family Support Worker to join our team. We take a strength-based approach , empowering children, young people, and families to address emerging worries and make positive, lasting changes. About the Role As an Outreach Family Support Worker , you will act as the lead professional in supporting families, coordinating with our multi-agency partners to ensure a whole-family approach is delivered. You will: Undertake Early Help Assessments with families. Develop tailored support plans to meet identified needs. Chair and coordinate Team Around the Family (TAF) meetings , monitoring progress and identifying further support where required. Work directly with children and young people, ensuring their voices, views, and worries shape the interventions and outcomes. This role is pivotal in helping families overcome challenges early, strengthening resilience, and improving the long-term outcomes for children and young people in Liverpool. About You We are looking for someone who is: Experienced in working directly with children, young people, and families. Skilled in assessing needs, planning support, and coordinating multi-agency input. Able to engage families in a supportive, empathetic, and empowering way. Confident in chairing meetings and ensuring collaborative working. In possession of a full UK driving licence and access to a vehicle (essential, due to travel across Liverpool).
