
Memory Assessment Nurse

- To work as part of the multi disciplinary team to screen for appropriate referrals
- To complete an assessment of clinical presentation and symptoms
- To share information amongst the multi disciplinary team and create appropriate and person centered care plans
- To refer for appropriate diagnostic procedures eg scans, neuropsychology, occupational therapy.
- To make independent diagnostic decisions based on consultation, history, presentation, pathology and/ or imaging results and to seek supervision from service medical support in complex diagnostic situations.
- To complete accurate records
- RMN E.N.B 998 OR City and Guilds 730 or equivalent.

- Supervision course.
- Course related to therapeutic skills.
- Evidence of continuous professional development.
- Substantial experience of working with people with dementia.
- Experience of working in the community or day hospital setting
- Experience of working with individuals with working age dementia.
- Can evidence ability to undertake assessment and treatment planning for individuals with complex presentations.
- Good written and verbal skills with patients, carers and members of multi-disciplinary team
- Effective time management, organisational and prioritisation skills.
- Able to work independently and as part of a team.
- Can demonstrate caring and compassionate attitude.
- A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
