Be part of a supportive team that values your skills and empowers you to deliver exceptional care from hospital to home.What will your responsibilities be?In this role, you will support individuals following hospital discharge by conducting timely reviews and participating in Decision Support Tool (DST) meetings to assess ongoing care needs. You may also be involved in Adult Safeguarding work and, after gaining experience, will be expected to contribute to the team’s duty rota, handling urgent cases and initial assessments as part of your responsibilities.Benefits:
