As part of this we encourage and provide opportunities for continued professional development and regular supervision to help develop nursing confidence, skills and career.This post will be based in Hedon with the expectation to work across the Holderness area due to service demand requirements. Due to the geographical nature of the post it is essential that you can drive and have access to a car.Main duties of the jobAs a key worker / care coordinator you will provide support and interventions to promote mental health recovery. Establishing therapeutic relationships with service users and their family / carers.
  • Develop and deliver recovery focused care in collaboration with service users and their family /carers.
  • Provide information and psycho education surrounding mental health conditions and medication.
  • Uphold legal frameworks which underpin service delivery.
  • Assess risks, the management of risks and effectiveness of interventions.
  • Deliver recovery based and risk management interventions with complex clients.
  • Signposting / referral to external services when required.
  • Develop good working relationships with neighbouring teams to enable effective co-working and responsive transfers of care if necessary.
  • Act as duty officer on a rota basis
  • Involvement in Clozapine Clinic
  • Administration and monitoring of medication.
  • Supervise other members of the team and students.
  • Leadership to support the team day to day and in meetings.

  • Demonstrable experience of mentoring pre-registration students
  • Full understanding of relevant policy, legislation, drivers and their application to clinical and service area, e.g. The Mental Health Act, Mental Capacity Act.
  • IT skills for learning systems required for role e.g. electronic patient record, clinical stock ordering systems, incident reporting systems.
ExperienceEssential
  • Previous community based experience or band 6 experience transferrable to community setting Experience of leading on complex care delivery
  • Experience of leading on complex care delivery
  • Evidence of promoting/supporting active user/carer involvement/participation
Desirable
  • Leadership/management experience which has had a positive impact and created change within the service delivery/practice
  • Ability to effectively Chair meetings

  • Ability to effectively transport self between multiple addresses/venues on a daily basis including transporting, clients, equipment & clinical stock required for treatment.
