- GPHC Registration as a Clinical Pharmacist
- Masters degree in pharmacy (MPharm) or equivalent
- Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience
- Completed CPPE pathway
- Independent prescriber
- Enrol on or have completed the CP Advanced
- Practice Programme/ Portfolio. Completion is a
- requirement of the role.
- Member of the Royal Pharmaceutical Society
- Fellow of the Royal Pharmaceutical Society
- Minimum of 2 years postqualification experience
- Experience of supervising a clinical team
- Experience of leadership
- Experience of working with patients in a community setting.
- Experience of managing common acute and long-term conditions.
- Ability to work as part of a team and autonomously
- Excellent written and verbal communication skills
- Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences including staff, managers and patients
- Ability to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
- Ability to work well under pressure and meeting deadlines.
- Ability to prioritise when problem solving and identify deviations from normal pattern
- Produce timely and informative reports.
- Gain acceptance for recommendations and influence/motivate/persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers.
- Ability to follow legal, ethical, professional and organizational policies/procedures and codes of conduct
- Demonstrates accountability for delivering professional and direct service provision
- Ability to recognize personal limitations and refer to more appropriate colleague(s) when necessary.
- Ability to maintain a high level of confidentiality and discretion at all times
- Experience of working across agency boundaries and in local partnerships
- In depth therapeutic and clinical knowledge and understanding of the principles of evidencebased healthcare
- An understanding of the nature of general practices
- An understanding of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
- Excellent interpersonal, influencing and negotiating skills
- Excellent IT Skills and experience of computer-based GP clinical systems (EMIS, AccuRx)
- People/line management experience
- Experience of working in a primary care setting
- Experience of contract monitoring
- Experience of performance management
- Able to collect, store and analyse data for service evaluation purposes
- Self-motivated
- Proactive self-starter
- Adaptable
- Full Driving Licence and access to a car
