This post is an exciting opportunity for a Senior Pharmacy Technician to work as a member of the Virtual Frailty Ward Team within North Tees & Hartlepool NHS Foundation Trust. The role will focus on promotion of safe, rational, effective, and economic use of medicines used in Virtual Ward patients. The role will include conducting medicine reconciliation for these patients, ensuring that safe and effective clinical interventions are made as appropriate. The post holder will provide pharmaceutical advice to members of the Virtual Ward Multi- disciplinary Team (MDT) with the support of the clinical pharmacist team. The post holder will be expected to be an active member of the virtual ward MDT, providing feedback to virtual ward rounds when required. There will be an expectation that the post holder will support in delivery of training to members of the virtual ward MDT on the safe use of medicines and medicines optimisation.   • Registered with the GPhC
