What you will do:
  • Deliver high-quality, patient-centered care focused on rehabilitation and recovery to prepare patients for discharge home or into long-term settings.
  • Conduct assessments and design personalised treatment plans for individuals in a bed-based rehabilitation setting.
  • Collaborate with a motivated team to provide daily rehabilitation interventions and contribute to multi-disciplinary discussions for optimal patient outcomes.
  • Ensure effective discharge planning through communication with patients, families, and other healthcare professionals.
  • Drive to various care home settings as needed (car driver essential).
