An opportunity has arisen for an enthusiastic and organised person to join our locality team as one of our Health and Social Care Co-ordinators.The role is pivotal to bring services together in order to provide seamless care for frail and vulnerable individuals living in Weymouth and Portland. It is an exciting opportunity to be at the forefront of developments in Community Services.Applications from candidates that require current Skilled worker sponsorship to work in the UK are unfortunately ineligible to apply for this position.Employment in this post requires a Standard Disclosure and Barring Service (DBS) check which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.Main duties of the jobThe successful applicant will need to have excellent organisation skills and be a tenacious and skilled communicator as the role is integral to the development of integrated and co-ordinated services.You would need to have previous experience in a health or social care environment and have excellent administration skills to co-ordinate communications and meetings between primary care, community services and hospitals (physical and mental health), local authority, acute trusts and voluntary services.You will work alongside clinicians, patients, carers, statutory and voluntary agencies connected to the care of the individual patient to proactively support an effective assessment and the development of an integrated personal plan of care, to assist in avoidance of unplanned admissions and facilitate timely hospital discharge.About usAt Dorset HealthCare we're on a mission to empower people to make the most of their lives through our outstanding, joined-up healthcare services.
