This role supports adults dealing with physical health conditions and older people with dementia, including facilitating discharges from mental health wards. You’ll also be providing longer-term support to those receiving adult social care living in the west of the city or whose GP is part of the primary care network in the West. – Perform comprehensive assessments of older adults and individuals with physical health conditions.
– Coordinate and support the discharge planning process for older adults from mental health wards.
– Provide ongoing care and support management for adults in receipt of social care.
– Collaborate with healthcare providers in the West area of the city to ensure cohesive primary care support.
– Attend face-to-face team meetings every four weeks to discuss strategies and share insights.
– Adapt supervision to match both your needs and those of your clients, promoting a flexible support structure.Requirements of the Social Worker:
– Degree or equivalent in social work
– Current Social Work England registration
