- Direct work with Looked After Children, Care Leavers and Homeless Young People aged 16 – 25, developing their pathway into independence
- Take responsibility on a day-to-day basis for your individual caseload
- Home/ Community visits
- Support to improve life chances and outcomes
- Offer assessment and support to young homeless people
- Support to access accommodation, employment, education
- Support with independent living skills such as budgeting, cooking, cleaning
- Assess and implement plans to address behaviour
- Assess their individual needs with a course of action
- Prepare, produce, implement and evaluate plans
- Prepare for and participate in decision making forums
- Work with partner agencies and colleagues to provide appropriate services
- Maintain appropriate records of services provided to service users
- Attend reviews, case conferences, pathway planning and meetings where appropriate
- UK Driving Licence, access to a vehicle and business insurance
- Recent extensive experience working with young people leaving care
- Ability to undertake assessments and review programmes, including the provision of pathway plans
- Knowledge of issues facing young people leaving care or becoming homeless
- Ability to engage with resistant Young People
- Ability to visit clients in the Bolton area
- Excellent written and verbal communication skills
- You Must Have Right to Live & Work in the UK
- Enhanced DBS registered to the update service
