If so an exciting opportunity to be part of our multidisciplinary team working across four practices to deliver coordinated care to our patient community.Previous applicants need not apply.Please note we reserve the right to close applications early.Main duties of the jobThe post holder must be an experienced Healthcare Assistant in general practice, who can support in managing specific cohorts of patients with a range of acute and chronic medical conditions. They will work as part of the team, delivering care within their scope of practice to patients within the surgery and conducting home visits. They will deliver and assist clinical staff in the provision of treatment, preventative care, health promotion and patient education. They will carry out baseline observations such as pulse oximetry, blood pressure, phlebotomy, BMI, temperature as directed and record findings accurately. They will support with management of chronic disease, delivering opportunistic health promotion where appropriate. The post holder may complete basic care plans.About usThe role will support four practices within the Glossop area, with approx. 33,350 patients.
