  • Supportstaff in applying the NHS CHC Framework.
  • Jointly manage complex cases with adult care practitioners.
  • Ensure guidance, training, resources, and practice standards are in place.
  • Monitor CHC activity using appropriate systems.
  • Represent Lincolnshire Adult Care on panels,supportdispute resolution, and maintain effective relationships with NHS partners.
