Are you a dynamic, innovative, and enthusiastic Advanced Clinical Practitioner with a passion for supporting individuals with memory difficulties? Do you want to be part of a pioneering initiative that is reshaping Dementia and Frailty services across Birmingham?We are excited to offer a unique opportunity to join a newly funded project under theBirmingham Fairer Future Fund, aimed at tackling health inequalities through culturally sensitive and collaborative care pathways. Essential Criteria:
  • Registered healthcare professional with ACP qualification.
  • Experience in dementia care or memory services.
  • Strong commitment to reducing health inequalities.
  • Excellent communication and stakeholder engagement skills.
  • Ability to work autonomously and collaboratively in a multi-agency environment.
