The team at the Glebe Practice Saxilby are looking to recruit an enthusiastic Practice Nurse to work with us to cover an 18-month maternity leave. There may be an opportunity for a permanent post at the end of the fixed term contract.Our Practice -Situated in Saxilby Lincolnshire We have a list size of 8200 patients with a varied patient demographic. We are a GMS, Emis web Practice.Main duties of the jobJob Plan -To support patients to improve and maintain their mental, physical, behavioural health and well-being by using behaviour change interventions, such as undertaking health checks (physical and mental). To support health action planning, as part of Learning Disability and Severe Mental Illness physical health checks, with the support of practice clinical leads and community specialist teams.To undertake Chronic disease management (e.g. asthma, diabetes, COPD). Immunisations and vaccinations (including travel vaccines). Cervical cytology (smears), wound care and dressings, Health promotion and lifestyle adviceTo maintain effective infection control measures in line with local guidelines and practice policy. To participate in developments within the practice / network and support QOF / DES requirements.To be registered with the NMC. Work in accordance with the NMC Code of Conduct and Scope of Practice for Practice Nurses Ensure accurate notes of all consultations and treatments are recorded in the patients notes and on computers, as per NMC guidelines.Qualifications
  • NMC-registered Practice Nurse with recent primary care experience
  • Confident in managing long-term conditions and working independently
  • Skilled in using clinical systems Emis desirable but not essential)
  • Compassionate, organised, and a strong team player
