- To be a Key Worker for their caseload ensuring everyone receives a high standard of personalised care, working to coordinate care and provide easy access to services and ongoing physical and emotional support for patients their carers and families.
- To be the patient advocate within the site-specific Multi-disciplinary Team (MDT).
- Demonstrates leadership through clinical expertise delivering high standards of personalised care by acting as a key worker, identifying needs through holistic needs assessment and co-creating a care and support plan with people living with cancer.
- Deliver Nurse Led Clinics utilising Holistic Needs assessment.
- To establish working relationships and communication links between primary secondary tertiary and independent care providers to improve care for patients and provide specialist clinical expertise, advice and support when required.
- Ensure that professionals adhere to the implementation and monitoring of relevant clinical guidelines, policies and procedures within their scope of practice and speciality.
