
Social Worker – Assessment Team

– Engage with individuals, families, carers, and other professionals to gather relevant information.
– Make professional judgments regarding safeguarding and protection concerns and initiate appropriate safeguarding procedures when necessary.
– Develop and implement person-centred care and support plans.
– Liaise and collaborate with partner agencies including health services, education, housing, and the police to coordinate care and support.
– Maintain accurate, timely, and confidential case records.
– Provide advice and information to service users and their families about available services and support options.
– Attend and contribute to case conferences, multi-agency meetings, and court proceedings as required.
– Work within legislative frameworks such as the Children Act, Care Act, or Mental Health Act.
– Participate in supervision, training, and professional development activities.Qualifications and Experience:– Qualified Social Worker with recognised accreditation, such as HCPC registration in the UK.
– Experience in conducting social work assessments, preferably within a statutory or local authority setting.
– Sound knowledge of relevant legislation, safeguarding procedures, and social care practices.
– Strong communication and interpersonal skills.
– Ability to work independently and as part of a multidisciplinary team.
– Resilience and the ability to manage complex and emotionally challenging situations.Desirable:– Experience in working with children and families or adults with complex needs.
– Additional training or qualifications in safeguarding, risk assessment, or mental health.Working Conditions:This is an office-based role with frequent visits to service users’ homes or other community settings.
