What you will do:
  • Conduct comprehensive bedside assessments at hospitals to determine patients' needs.
  • Travel to complete home assessments, empowering individuals to achieve greater independence.
  • Engage in care act assessments, promoting a tailored approach to support each person’s trajectory towards reablement.
  • Collaborate with our Homefirst Neighbourhood team to find creative solutions in the discharge to assess process.
  • Attend occasional meetings in the office, while enjoying the flexibility of completing paperwork from home.
As a qualified Occupational Therapist, registered with HCPC, you are required to have two years of post-qualification experience.
