  • Patient Care:Deliver general practice nursing care, including chronic disease management, immunisations, wound care, and health screenings. Occasionally undertake home visits as needed.
  • Health Promotion:Educate and advise patients on health promotion, disease prevention, and self-management strategies for long-term conditions.
  • Clinical Procedures:Perform clinical procedures such as venepuncture, ECGs, and cervical screening, ensuring adherence to clinical guidelines.
  • Care Planning:Develop, implement, and evaluate individualised care plans for patients, ensuring a patient-centred approach.
  • Team Collaboration:Work as part of a multidisciplinary team, liaising with GPs, healthcare assistants, and other healthcare professionals to ensure comprehensive patient care.
  • Record Keeping:Maintain accurate and up-to-date patient records, ensuring confidentiality and compliance with data protection regulations.
  • Professional Development:Engage in continuous professional development, attending training sessions and keeping abreast of current nursing practices and guidelines.

  • Qualified in chronic disease management for Diabetes, Asthma, COPD and CHD/CVD
  • Wound dressing and leg compressions
  • Cervical screening qualification
  • Vaccinations and Immunisation qualification
