
Practice Nurse

- Patient Care:Deliver general practice nursing care, including chronic disease management, immunisations, wound care, and health screenings. Occasionally undertake home visits as needed.
- Health Promotion:Educate and advise patients on health promotion, disease prevention, and self-management strategies for long-term conditions.
- Clinical Procedures:Perform clinical procedures such as venepuncture, ECGs, and cervical screening, ensuring adherence to clinical guidelines.
- Care Planning:Develop, implement, and evaluate individualised care plans for patients, ensuring a patient-centred approach.
- Team Collaboration:Work as part of a multidisciplinary team, liaising with GPs, healthcare assistants, and other healthcare professionals to ensure comprehensive patient care.
- Record Keeping:Maintain accurate and up-to-date patient records, ensuring confidentiality and compliance with data protection regulations.
- Professional Development:Engage in continuous professional development, attending training sessions and keeping abreast of current nursing practices and guidelines.
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team.
- Knowledge of infection control systems to achieve compliancy.
- Experience in maintaining cold chain compliancy

- Experience in a primary care or general practice setting is desirable.
- Knowledge of Long term management for Diabetes, Asthma, COPD and CHD/CVD
- Experience of emis clinical system
- Registered Nurse with the Nursing and Midwifery Council (NMC).

- Wound dressing and leg compressions
- Cervical screening qualification
