Liverpool City Council is looking for a dedicated and experienced Social Care Assessor to join their Adult Social Care & Health Team. This is a fantastic opportunity to work within a strengths-based, reablement-focused approach that places the person at the centre of care.🔍 Your Role: As a Social Care Assessor, you'll be part of a professional and supportive team working to make a real difference in people's lives. You will:
- Carry out strengths-based assessments with adults and carers
- Develop and review outcome-focused support plans that promote independence
- Apply a reablement-first approach in all interventions
- Collaborate with families, health professionals, and partner agencies
- Manage your own caseload effectively and meet review timelines
- Maintain accurate records using Liquid Logic case management system
- Work across multi-disciplinary teams to achieve the best outcomes for service users
- Experience in social care and reablement settings
- A relevant social care qualification
- Proven experience working in a local authority or statutory/voluntary agency
- Confidence using Liquid Logic
- Strong understanding of adult social care legislation and safeguarding
- Excellent communication and interpersonal skills
- Ability to build effective partnerships and tailor support to individual needs
