What you will do:
  • Provide a responsive social work service tailored to the needs of adults, carers, and families, ensuring you follow statutory guidelines
  • Participate in assessments, care planning, and safeguarding activities, with support provided according to your level of experience
  • Promote personalised support and empower service users to choose and control their support plans
  • Manage a caseload, conducting assessments in collaboration with service users, carers, and multidisciplinary partners
  • Assist individuals in identifying how to meet long-term needs and aspirations using an asset-based approach
  • Build outcomes-focused support plans that align with the strengths-based principles of the Care Act
