Job Title: Social Worker - Assessment Team
**Location:** Stockton on TeesPosition Overview:
We are seeking a dedicated and experienced Social Worker to join our Children Assessment Team. The ideal candidate will have a strong background in child welfare, with a focus on assessing and addressing the needs of children at risk. As a member of our team, you will play a crucial role in evaluating the safety and well-being of children, conducting assessments, and developing plans to ensure their protection and support.Responsibilities:
Conduct comprehensive assessments of children and families referred to the Children Assessment Team.
Work collaboratively with families, caregivers, and other professionals to gather information and develop a holistic understanding of the child's situation.
Assess risks and protective factors to determine the level of intervention needed to ensure the safety and well-being of the child.
Develop and implement care plans tailored to the unique needs of each child, with a focus on promoting stability, permanency, and positive outcomes.
Provide support and guidance to families, offering interventions and resources to address concerns and improve functioning.
Monitor progress and evaluate the effectiveness of interventions, making adjustments as needed to achieve positive outcomes.
Collaborate with internal and external stakeholders, including law enforcement, schools, and community organizations, to coordinate services and support for children and families.Qualifications:
Bachelor's or Master's degree in Social Work or a related field.
State licensure or certification as a Social Worker (LCSW, LMSW, or equivalent).
Strong knowledge of child development, family systems, and trauma-informed care principles.
Excellent assessment and case management skills, with the ability to develop and implement effective care plans.
Ability to communicate effectively with diverse populations and build rapport with clients and stakeholders.
Strong organizational skills and the ability to manage a demanding caseload in a fast-paced environment.
