Advanced practitioner to join a team of 4 existing Nurses and Paramedics forming advanced practice team within a busy General PracticeThe post holder will undertake assessment and treatment of patients in the surgery who may present with complex and/or chronic illnesses; to determine a clinical diagnosis, treatment and appropriate discharge or onward referral as indicated.The role will involve a mixture of daily triage within a MDT triage hub (using the ACCURX patient contact system) , independent clinician clinic with a mix of phone consultation and some home visiting.Helston Medical Centre have run a triage system since June 24 which the Advanced Practitioners / Clinical Practitioners play a key part of the triage hub team, liaising with Medical and clerical colleagues to ensure the safe management of patient demand against clinical capacity and service availability.The post holder will be a source of clinical expertise to the wider practice team, supporting ,medical, nursing and HCA colleagues with any clinical situations. Confident to work independently and as part of the wider clinical team.The post holder will contribute to meeting the identified Key Performance Indicators within the time frames identified.Main duties of the jobTo independently undertake highly specialised clinical assessments, diagnose, treat and discharge patients with a wide range of minor illnesses and chronic diseases including management of complex casesTo ensure patients are fully involved in the design and implementation of their treatment plans.To ensure effective coordination and liaison with multidisciplinary teams across Primary and Secondary Care Services, statutory and voluntary disciplines/groups in order to provide a seamless service to the patients and their carers and prevent hospital admission where urgent care is required.refer appropriately to in-house GP where a second opinion is required.adapt styles of communication appropriately in order to promote empowerment, self-management and health education for patients.attend clinical case conferences , review and clinical meetings as required.To be a source of clinical expertise to less experienced staff within the team.delegate duties/tasks to other members of staff where appropriate.Provide specialist advice relating to minor illness and/or injury to local healthcare teams, patients, relatives and carers.identify actual or potential clinical risks and take appropriate actionwhen required, the appropriate reporting structure Service delivery responsibilities.ensure that patient records and documentation are maintained in line with NMC or other relevant standards.facilitate onward referral for specialist opinion as requiredAbout usAs a growing organisation across several sites, we believe we can enshrine a model of General Practice that allows us to provide the best of both worlds, of smaller practice teams with the benefits and support of a larger structure.
