Hours: Full-Time or Part-Time (Core hours: Monday to Friday, 9am–5pm)We are looking for an experienced and dedicated Community Psychiatric Nurse (CPN) to join the Early Intervention in Psychosis team based at Hexagon Tower. This is a unique opportunity to make a real difference by supporting individuals aged 14 to 65 experiencing their first episode of psychosis. You will coordinate care, deliver interventions, and promote early recovery, aiming to prevent hospital admissions and improve outcomes for service users and their families.What You Will Do:Conduct holistic assessments of mental health, risk, and social circumstances.Deliver CBT-based interventions, psychoeducation, and family-focused support.Coordinate care under the Care Programme Approach (CPA), ensuring timely reviews and collaborative planning.Administer and monitor medication in line with clinical guidelines.Provide home visits and community-based support to service users.
