Main ResponsibilitiesAs a Social Worker in this team, you’ll:Have a passion for supporting adults to achieve their full potential and want to gain further experience in improving outcomes.Have the chance to support adults from the very start of their journey, reviewing their goals and ambitions.Continue to promote independence and opportunities, ensuring that these have positive outcomes.Undertake assessments, support planning, review and case management to support adults with a learning disability and/or autism ensuring that they, their families/carers and the people important to them are supported to live the life that they want to live.
