- Independently assess, diagnose, and treat patients in multiple settings, GP Practice, own home, care home, across the Practice area
- Work side-by-side with our multidisciplinary teams to keep care patient-focused and community-centred
- Provide expert clinical advice to patients, carers, and colleagues
- Initiate referrals and help uphold clinical excellence in a fast-moving environment
- Bring autonomy and confidence to decision-making in complex, undifferentiated cases
- Ensure the clinical environment is prepared according to health and safety, infection control policy and report any incidents to service lead.
- Monitor and maintain supplies of materials and equipment within the care environment.
- Report all accidents / incidents/ complaints relating to patient care to the service lead.
- Work in accordance with organisation policies/procedure.
- Manage own clinical workload responding effectively to patient need and ensure patients choice and ease of access to the service.
- To mentor and offer clinical supervision to other practitioners and clinical advisors in developing and maintaining clinical skills to effect change. Contribute to plans that reflect changes in the service.
- Working across primary, secondary, and social care services.
- Support the delivery of key quality indicators and use to promote continual improvement of the service within a clinical governance framework.
- Monitor and evaluate practice through clinical audit and formal evaluation processes, plus research and development relating to the evolving role.
