The role will work as part of a multi-professional team providing palliative nursing care interventions for patients and their families in the community. Responsible for the observation of patients and the provision of care in accordance with the management plans under the supervision of a Team Leader, Registered Paramedic/Nurse or Clinical Nurse Specialist for patients in their homes.Main duties of the jobWe particularly encourage applications from disabled candidates, candidates from ethnic minority groups, LGBTQ+ as well as non-binary candidates and those who have experienced mental health issues.
