– Lead safeguarding enquiries under Section 42 and undertake meaningful assessments in line with the Mental Capacity Act.
– Manage a diverse and autonomous caseload, typically consisting of 16–20 individuals, with a focus on prioritising urgent visits and strategic risk planning.
– Empower individuals through a strengths-based, person-centred approach that promotes their choice and control over care decisions.
– Collaborate effectively with health providers, housing organisations, voluntary groups, and multi-disciplinary colleagues to ensure holistic care.
– Comply with internal key performance indicators, maintaining precise case records promptly, and adhering to statutory reporting obligations aligned with legal and organisational standards.
