- Support individuals with personal care, medication, cooking, and household tasks
- Enable involvement in the local community and promote independence
- Support with making and attending health care appointments
- Communicate with medical professionals and implement support plans and risk assessments
- Maintain clear and accurate records of support provided
- Communicate effectively with the team, family members
- Work independently and as part of a supportive team

- Enhanced PVG required (we will support the application process)
