- Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with undiagnosed and undifferentiated illness and injury (children and adults) often with complexity.
- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
- Triage telephone referrals, calls from external clinicians, patients and others into a range of services appropriately as required.
- Arrange and interpret investigations within the remit of the Urgent treatment service such as blood tests and X-rays.
- Acts as an autonomous practitioner within agreed protocols guided by organisational policies and within Scope of Professional Practice and governing body.
- Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
- Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and local protocols, and within scope of practice.
- Work with patients in order to support compliance with and adherence to prescribed treatments.
- Adapt to clinical services which may be new, innovative and challenging at times with a flexible approach to learning the skills required to perform optimally in the role.
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.
- Maintain competence in clinical practice in line with the practitioners professional regulatory body in line with regulations and revalidation requirements.
- Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment including patients that are receiving bad news.
- Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
- Maintain effective communication within the practice environment and with external stakeholders.
- Act as an advocate for patients and colleagues.
- Ensure awareness of sources of support and guidance such as online/electronic evidence-based leaflets, signposting patients to relevant services and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate.
- Coordinating care for complex patients with consent to help them navigate the often complex healthcare system and support them through effective communication to remain well at home.
- To communicate with external stakeholders and other parties to share a positive image and message of Tower Hamlets GP Care Group.

- Experience of working independently with patients with urgent need
- Able to prescribe medications under scope of practice according to formulary
- Ability to interpret blood tests and X-rays
- MSc in Advanced Practice/Dip He (N) Advanced Practice
- Knowledge of the 4 pillars of advanced practice
