GMMH is delighted to be able to offer a full-time position of mental health homeless practitioner within the Manchester Mental Health and Homeless Team.The offers are open to anyqualified Mental Health Nurses, Social Workers and Occupational therapists who have active registration within the relevant body.The successful candidate will work to support and strengthen existing links with support services and mental health treatment services within the city of Manchester. The post holder will be supported by a well-established, specialist multi-disciplinary, trauma informed homeless mental health team and will work with individuals who are experiencing mental health problems. The role of the team is to work with a range of partnership services and homeless individuals to assess and identify the appropriate treatment pathways to address these unmet needs.Please feel free to contact the manager if you are interested in the post.Main duties of the jobThe post holder will provide community based engagement and assessment to clients with complex mental health and substance misuse (dual diagnosis) needs in partnership with mental health and addiction services.The post holder will be part of the Manchester Homeless Mental Health Team and will work alongside council staff, drug and alcohol workers, police and homeless outreach workers providing mental health support to rough sleepers both through drop-in clinics at day centres or directly on the street whilst working alongside outreach workers.The post holder will be responsible for the assessment of mental health needs for homeless individuals who are not engaged with mainstream mental health services.
