The post holder will assist in the provision of an efficient, effective and comprehensive administrative service supporting the ward based team. The post holder will be expected to work alongside Patient Flow / Integrated Discharge Team as well as external partners to facilitate flow of patients through the wards and facilitate discharges. The post holder will adopt the use of current discharge processes to ensure safe, timely discharges.They will report to the clinical care coordinator for the integrated discharge team.   • Update of Patient Discharge Tracking List (OPtica)
  • Access patient details using the trust electronic systems and Trakcare
  • Ensure patient flow is maintained and support electronic system update re patients location
  • Monitoring of transport, bookings and ensuring patients are discharged in timely manner, and using appropriate transport methods
  • Liaise with patient flow team regarding expected patient discharges and promoting the use of the Transport Hub.
  • Escalate issues to Clinical Care Coordinators for them to review and aid as required to help facilitate patient flow.
