  • Conduct annual reviews and manage non-complex care cases in collaboration with individuals and their families.
  • Perform Community DoLS assessments to ensure compliance with relevant legislation.
  • Undertake occasional duty tasks to support the team and enhance service delivery.
  • Engage collaboratively with various professionals and stakeholders to advocate for individuals with learning disabilities.
  • Keep thorough, accurate records of all case work and assessments.
