- Provide general medical services to registered and temporary patients.
- Provide appropriate management of acute, chronic, and complex conditions.
- Ensure accurate, timely, and complete documentation of patient records.
- Prescribe in accordance with practice and national guidelines, ensuring safe repeat prescribing and medicines optimisation.
- Undertake appropriate investigations, referrals, and follow-up.
- Support the practice in meeting Quality and Outcomes Framework (QOF), Investment and Impact Fund (IIF), Direct Enhanced Services (DES), Local Enhanced Services (LES), and other contractual requirements.
- Work within Care Quality Commission (CQC), the General Medical Council (GMC), NHS England, and practice governance frameworks.
- Participate in significant event reviews, complaints handling, and clinical audits.
- Support continuous quality improvement in the practice.
- Maintain confidentiality and comply with GDPR and information governance requirements.
- Undertake patient consultations via face-to-face, telephone, online, and home visits as appropriate.
- Provide health promotion, screening, disease prevention and advice in line with national and local public health initiatives.
- Collaborate with a multi disciplinary team including practice nurses, HCAs, care navigators, pharmacists, paramedics, physiotherapists, and others.
- Participate in practice clinical meetings, MDTs, and case reviews.
- Build and maintain effective communication with patients, carers, and colleagues, demonstrating empathy, clarity, and professionalism at all times.
- Work as part of a large, inclusive and forward-looking training practice serving 32,000+ patients.
- Contribute to supervision and training of GP trainees, foundation doctors, and medical students.
- Support the practice's involvement in PCN initiatives through The Five Parks PCN.
