- Direct Work with Individuals: Building strong and trusting relationships with individuals, conducting thorough assessments, and developing individualized care plans that address their unique needs and challenges.
- Case Management: Managing a caseload of individuals, ensuring their wellbeing and safety are prioritized, and coordinating services with other professionals and agencies.
- Support and Advocacy: Providing emotional support, guidance, and advocacy to individuals and their families, empowering them to make informed decisions about their care.
- Multi-agency Collaboration: Working effectively with other professionals, such as health services, housing providers, and community support organizations, to ensure a coordinated approach to care.
- Promoting Wellbeing: Encouraging individuals to develop coping mechanisms, build resilience, and work towards their personal goals.
- Promoting Inclusion: Advocating for the rights and needs of individuals with learning disabilities, ensuring they have equal opportunities to participate in their communities.
- Social Work Qualification: You must be a qualified Social Worker.
- Social Care England Registration: Essential for this role.
- Updated DBS: Essential for working with vulnerable individuals.
- Experience in Working with Adults with Learning & Disabilities: Demonstrated experience working with individuals with learning disabilities and a strong understanding of their unique needs and challenges.
- Excellent Assessment and Case Management Skills: Proven ability to conduct comprehensive assessments, develop effective care plans, and manage complex cases effectively.
- Strong Communication and Interpersonal Skills: Ability to build rapport, listen actively, and communicate effectively with individuals and families.
- Commitment to Safeguarding Vulnerable Adults: Passion for protecting individuals and ensuring their rights and well-being are paramount.
