We are looking to recruit an Advanced Nurse prescriber who will work within the South West Herts Enhanced Primary Care Monitoring service to provide support and intervention to children and young people with ADHD (as well as co-morbidities) between the ages of 6 and 18.The post holder will be confident in completing ADHD medication reviews utilising specialist nursing skills and using clinical interview techniques in order to ascertain a childs response to medication and manage potential side effects. They will be expected to be confident in ascertaining potential symptoms of co-morbidity such as ASD, mental health needs or physical health concerns and document information accurately and refer to appropriate local services.You will be experienced in titrating ADHD medication in children and young adults and confident with the formulary.Main duties of the jobThis is a clinic-based role, and you will carry out holistic assessments of patients referred as appropriate by HPFT into this primary care monitoring service. Clinics are based within the 4 localities of West Herts, Dacorum, St Albans, Hertsmere and Watford and 3 Rivers, this role will be based across 2-3 sites.Apply knowledge of behavioural management strategies and evaluate responses to these and medication.Ability to communicate orally, electronically and in writing with children, young people, and families.Correspond with GP supervisors in the team, multi professional agencies, families, children services and voluntary/ statutory services. Attend weekly MDT meetings with senior team to discuss and review caseload regularly.
