- Play a key role in the pharmaceutical care of older people.
- Collaborate closely with the MOE pharmacy team and wider multidisciplinary team (MDT) to optimise patients' medicines and care.
- Support and develop other members of the pharmacy team
- Benefit from opportunities for service improvement, independent prescribing training, delivery of education and training and other personal and professional development initiatives.
- MOE ward rounds in the Medical Assessment Unit and in the specialist MOE admissions unit (ward 72)
- Downstream ward multidisciplinary team meetings / ward rounds
- Service development projects aimed at improving patient care and modernising service delivery.
- A Master of Pharmacy Degree (or equivalent) and General Pharmaceutical Council (GPhC) registration.
- Completion of or have equivalent competence mapped to the
