We are looking for a qualified practice nurse ideally with experience.   • Provision of cervical cytology, childhood and adult immunisations, and travel vaccinations
  • Supporting chronic disease management (e.g. asthma, COPD, diabetes, hypertension)
  • Promoting health education and self-care
  • Providing travel health advice and immunisations
  • Contributing to QOF, enhanced services, and practice development initiatives
  • Work within the EMIS Web clinical system to record consultations, manage recalls, and contribute to clinical audits.
  • Collaborate with GPs, pharmacists, practice nurses, HCAs and external services.
  • Support the practice in meeting QOF, IIF, and other long-term condition targets.
  • Provide ongoing education and lifestyle advice to empower patients in self-management.
  • Support service development and audit quality improvement initiatives.

  • Travel Vaccine qualification (e.g., diploma) is desirable but not
