To be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients.Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population.The Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, NHS health check clinics, cervical cytology and childhood immunisations as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure.Main duties of the jobThe core responsibilities of the Practice Nurse are outlined below. Occasionally, tasks outside this list may be required if within the reasonable scope of the role.
  • To undertake the Lead role of Infection Prevention Control (IPC) and work to the IPC policy.
  • Assess patient needs to ensure appropriate, evidence-based care is given, implement, and evaluate treatment plans for patients with chronic conditions.
  • Identify and support patients at risk of long-term illness, provides routine care aligned with NICE and NSF guidelines, and manage asthma and COPD.
  • Review medication to improve compliance.
  • Coordinate travel vaccinations.
  • Maintain accurate clinical records in line with legislation.
  • Effective use of read codes and chronic disease registers is essential.
  • Prioritise health issues and intervene appropriately.
  • Support in clinical emergencies.
  • Assess and refer patients with mental health needs according to national guidelines.
  • Deliver adult and child vaccination programmes.
  • Provide opportunistic health promotion (e.g., during new patient checks)
  • Assess and care for uncomplicated wounds.
  • Maintain the emergency drug and equipment log
  • Maintain fridge temperature log as per the cold chain policy.
