RESPONSIBILITIESKey areas of responsibility will include:Provide treatment and care in conjunction with the GPs or independently as appropriate and according to practice policy and guidelinesTake responsibility for own clinics and time managementReview and filing of own test results and those relating to chronic diseaseAdvise patients on general health and minor ailments with referral to the GP as necessaryImmunisations and vaccinations including childhood immunisationsHoliday vaccinations and travel adviceVenepunctureFirst Aid for presenting emergenciesDiagnostic tests such as ECG, urinalysis, swabs, blood testsResponsibility for the nursing management of patients with chronic disease/long term conditions including diabetes, CHD, CVA, asthma, COPD, CKD, mental health, learning disabilities, autism and hypertension.To initiate, develop and implement protocols in relation to patients with chronic diseaseTo carry out cervical cytologyCarry put new patient health checks, well man and well woman checksActively promote health education and provide smoking cessation and weight management adviceProvide family planning and contraception adviceAct as chaperone where requiredTo promote and run health campaigns as appropriateEnsure the drugs cabinet and doctors bags are monitored and kept up to dateTo carry out risk assessments in accordance with COSHH regulationsKeep up to date with practice policies and procedures, especially in relation to recording systems, administration of drugs, child protection, confidentiality, health and safetyAdhere to infection control procedures and ensure practice policies are up to dateCleaning of equipment and disposal of wasteTo be competent in the use of the computer ensuring patients details are correctly input in accordance with practice guidelinesTo actively contribute and oversee QOF targets directly relating to the practice nurse roleTo provide vaccination to patients in their own home where requiredSupport PCN initiatives when required3. AUDIT & RESEARCHCollect routine data and participate and initiate clinical auditTo implement audit and research findings to improve clinical practiceTo maintain disease registers4. RELATIONSHIPResponsible to the Partners for clinical issues and the Practice Manager for administrative issuesTo work closely with other members of the nursing team, practising good teamwork at all timesLiaise with other team members e.g. admin and reception staff, District Nurses, Health Visitors, Community Matrons, School Nurses etc.Liaise with secondary care and social services to ensure safeguarding, quality and continuity of care5. ACCOUNTABILITYMaintain own annual registration with the Nursing and Midwifery Council and work within the scope of the NMC guidelinesBe personally accountable for own practice and promote a professional approach to patients and staffAcknowledge any limitation in knowledge and competence and practice within own limits at all times, highlighting any training needs to the Practice ManagerMaintain accurate records in line with NMC guidelines including data collection, completion of templates and read codingMaintain own continuous professional development and ensure revalidation and PDP is kept up to date in line with the requirements of the practice and the NMCMaintain own car insurance ensuring cover is in place for work related activities6.
