Job Category: Social CareJob Description:Family Practitioner | 8646 | Permanent Contract | 37 hours per week (Part-time considered) | £34,434 to £36,363 per annum | Grade H | County Hall Norwich and Elizabeth House, Dereham1 FTE North, East & Broadland1 FTE West & BrecklandWe are recruiting a case holding Family Practitioner to work within our Children with Disabilities, West and Breckland and North, East and Broadland team.The successful candidate will manage and be accountable for a caseload of vulnerable children, young people and families to provide targeted support and challenge as part of an integrated approach to support families where their child has a significant or complex disability.About us:Our Children with Disabilities service provides support to children and young people with substantial and permanent disabilities 0-18 years of age. We have 3 teams which cover Norfolk and they are multi-disciplinary consisting of social workers, occupational therapists, occupational therapist assistant practitioners and assistant practitioners. We maintain strong links with internal and external partners to ensure a holistic response to the needs of the children and families we are working with.You will have :
  • Minimum of two years of relevant experience of direct work and interventions with children, young people and families, preferably educated to degree level or NVQ Level 4.
  • A willingness to undertake specific relevant training to the role within the first year.
  • Experience of best practice.
